Request account deletion

Use this page to request deletion of your MediDates account and associated personal data. Do not include medical notes, diagnoses, files, images, or appointment details in this form.

Start deletion

Enter the account email first. MediDates sends a verification token before any account data is processed for deletion.

Verify request

After you receive the token, paste it here to confirm account ownership and continue the deletion request.

What happens after verification

Active sessions are revoked and live service data is targeted for deletion or de-identification.

Encrypted backups may retain deleted data until the rolling backup retention window expires.

This synthetic-stage deployment may require manual support follow-up if live deletion email delivery is not yet configured.
